In reality, however, well over half of people spend their final days in a hospital or nursing home.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Only a small proportion die in a specialized hospice facility (about 4.8 percent) or in a palliative care unit (about 6.2 percent). It is particularly distressing that many people die alone. In hospitals, this applies to about one in five people; in nursing homes, it is as high as one in three. There is also a great deal of fear surrounding this issue: About two-thirds of people worry that they will be left alone when they die.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><\/p>\n\n\n\n<h2 id=\"organisation\" class=\"wp-block-heading\"><strong>2. Outpatient hospice services support people where they live, that is, at home, in nursing homes or in hospital wards. Inpatient hospices are standalone facilities with a family-like atmosphere where terminally ill people spend the final phase of their lives, on average about 22 days.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">These services are provided by major charitable organizations such as Caritas, Diakonie, the German Red Cross (DRK), AWO, the Johanniter, and the Malteser, as well as by numerous smaller, often entirely volunteer-run local hospice associations. <a href=\"https:\/\/www.dhpv.de\/start.html\" data-type=\"link\" data-id=\"https:\/\/www.dhpv.de\/start.html\" target=\"_blank\" rel=\"noreferrer noopener\">The German Hospice and Palliative Care Association (DHPV)<\/a> serves as the national advocacy group for the hospice movement and connects the many individual organizations.<\/p>\n\n\n\n<h3 id=\"h-where-did-the-hospice-movement-come-from\" class=\"wp-block-heading\"><strong>Where did the hospice movement come from?<\/strong><\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The hospice movement in Germany emerged in the 1980s out of volunteer efforts with the goal of providing psychosocial, spiritual, and in some cases medical support to people who are dying. Much has changed since then. The number of outpatient hospice and palliative care services has more than tripled since 1996. Today, in Germany there are:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>about 1,500 outpatient hospice services<\/li>\n\n\n\n<li>Approximately 270 inpatient hospices for adults and 21 for children, adolescents, and young adults<\/li>\n\n\n\n<li>Approximately 330 palliative care units in hospitals, four of which are specifically for children and adolescents  <\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Despite these improvements, there remains a wide gap between what we hope for and reality. More than 100,000 people in Germany are actively involved in this way. A large portion of outpatient hospice services are even run entirely by volunteers, without funding from health insurance companies.    <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Before volunteers begin accompanying the dying, they undergo structured training. Afterward, they take on a wide variety of tasks, depending on what suits them best. Typical areas of responsibility in volunteer hospice work include:    <\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>End-of-life care<\/strong>: Giving your time, listening, and being there, whether at home, in a nursing home, in a hospital, or at an inpatient hospice<\/li>\n\n\n\n<li><strong>Grief Counseling<\/strong>: Continuing to support family members even after a person has died, for example, through grief support groups<\/li>\n\n\n\n<li><strong>Pediatric<\/strong> <strong>and Adolescent Hospice Care<\/strong>: Providing support to children and adolescents with life-limiting illnesses, as well as their siblings  <\/li>\n\n\n\n<li><strong>Making Wishes Come True<\/strong>: Supporting &#8220;W\u00fcnschewagen&#8221; projects that enable seriously ill people to take one last special trip<\/li>\n\n\n\n<li><strong>Behind-the-scenes support<\/strong>: If you prefer not to have direct contact with dying people, you can get involved in fundraising, event planning, or public relations<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">As varied as the tasks may be, the common goal is always to restore a sense of dignity, security, and humanity to the seriously ill and dying, as well as to their loved ones.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><\/p>\n\n\n\n<h2 id=\"ehrenamt\" class=\"wp-block-heading\"><strong>3. For outpatient end-of-life care, the training typically ranges from 80 to 120 hours; for inpatient care, it\u2019s usually around 60 hours. The courses aren\u2019t just about practical knowledge\u2014such as communication, setting boundaries, or legal basics like advance directives. Above all, they focus on your own inner attitude. You don\u2019t need any medical or nursing training for this.
